What you share on this form and in your meeting is treated in strict confidence between you and the Imam, except where safeguarding or legal duty requires a limited disclosure. The office may contact you only to arrange the appointment.

Submit a request for a time that suits you. The masjid will review availability; nothing is booked until you receive confirmation.

Calendar: after you send this form, you can add a provisional entry to your own Google Calendar, Outlook, or Apple Calendar. When the request is approved, the Imam or office may send the final online link or room details.
